My office-studio is located 2 minutes off the Langhorn exit on interstate 20 West in the Westview Studios building, in a large white church building.
I will text you instructions with the door code once we set an appt.
There is a waiting room across the hall from my door, I will get you from there at the time of your appointment.
If there is an emergency call Fulton County Crisis at 404.730.1600, Dekalb County Crisis at 404.892.4646, or call 911. If you are outside of this area, you may need to contact another local area crisis line or call 911 for immediate help.
Once we schedule a first session I will ask you to print out these three forms (Informed Consent, Intake Demographics, & Covid From). We will go over the informed consent in our first session, so don’t get to bogged down in all the details; and if you sign ahead, please put the date of our first session.
Please fill out the “one page” before our session. If our first session will be remote make sure you send me these 24 hours prior to our session time.
If you need me to talk with anyone (old therapist, psychiatrist, school counselor, etc) please print and fill out the Release of Information Form.
I offer phone and in person consultation surrounding opening your own practice, queer and trans youth and expressive therapy modalities among a few other arenas. Please feel free to reach out to touch base.
Monthly free talks at the Herbalista Health Fair since Feb 2015, and on mental health stigma in Atlanta/Decatur schools.
